Special Incident Reporting
California Code of Regulations (Title 17, Article 2, Section 54327), requires all service providers to report special incidents when they occur during the time the person served is receiving a service or support.
A Special Incident Report, or SIR, is a formal report that is filed with the regional center whenever a person served by the regional center is involved in an unusual event, such as a medication error, abuse, neglect, injury, death, or if the person served is a victim of a crime.
